All patient account data is stored on the website server so that the user can access the data through any device that supports
a web browser.
No data is stored locally on the user's device.
An Account that has not been accessed for more than one year may be deleted automatically.
The idea for the website diary originated from the printed diaries used by patients of Robert Lafitte Eller, MD in Greenville, SC.
The information on each Entry page form is based upon Dr. Eller's paper-form diaries.

Because there is no financial backing to support this website, it could disappear without prior notice.
As a precaution to patients, it would be wise to save the current reports as PDF files whenever a new entry is made.

This website was designed to assist patients receiving Botox injections to relieve the symptoms of Spasmodic Dysphonia and Vocal Tremor.
A diary of the symptoms that a patient experiences after a Botox injection
can assist the physician in determining the proper dosage for treatment.
There is no cost to a patient or doctor for access to this website.

There are two types of accounts: Patient and Doctor.
The Patient Account is used for recording symptoms of a patient receiving Botox treatments.
The Doctor Account enables a patient's doctor to view the symptoms recorded by the patient.

Patient Account
New Patient Page
A patient can create an account with a User ID that is unique to them but can be any string of characters to help the
patient account remain anonymous.
An easy-to-remember User ID value would be a First Name followed by the Initial letter of the Last Name with no blanks between them.
No personal information about the patient is saved on this website, other than their anonymous User ID and a
Password to sign into the account.
For security reasons, the Password is encrypted when saved and cannot be viewed by others, including the webmaster.
If the User ID is forgotten, it cannot be recovered or reset by the webmaster. If the Password is forgotten, it cannot be recovered or reset by the webmaster.

Botox Date Page
A Botox injection date is entered to establish a new period of the diary.
Patient symptoms are entered based upon the most recent Botox injection date.
When a new date is entered, there can be no more changes made to entries for any prior dates.
A new Botox injection date must be newer than the current Botox injection date and newer (or the same as)
the most recent Entry date.
If no Botox injection date is entered within 3 months after creating a new account, the account will be deleted due to inactivity.
Entry Page
A diary of the symptoms that a patient experiences after a Botox injection can assist the physician
in determining the proper dosage for treatment.
Entries are made in the diary at specified intervals.
The date of each entry is automatically determined on a schedule based upon the Botox injection date.
An entry is required for the day when a Botox injection is received.
During the first two weeks after a Botox injection, entries are made daily.
After the first two weeks, entries are made weekly.
An entry cannot be made for a future date.
An entry date may be skipped if no data is available for a given date.
However, entry dates that have been skipped can never be entered at a later time.
List Page
Details of each entry made for a given Botox injection date are shown in a list format.
If there is a mistake when making an entry in the diary, it can later be corrected.
However, changes can only be made to the current (most recent) Botox injection period.
Graph Page
A graph shows visually the trend of each type of symptom recorded for a given Botox injection date.
Reports
There are two types of patient reports: List and Graph.
Each type of report has a Print button to generate a special print copy which the patient could either send to a printer or save as a PDF file.
When a patient requests a print copy, there is a prompt for the patient to enter their name and birth date for printing.
The print copy contains the Name of the patient, the Birth Date of the patient and the Date of Injection.
As an option, the Name of the doctor can also be entered.
The name, birth date and doctor shown on the report are never saved anywhere on the website and are only used to generate the print copy.
After filling in the form information, there is a Print Report button that causes the report to be printed using a printer dialog box to select a printer or a PDF writer.
On the List page, if there are any comments available, they will be printed along with the data for symptoms.
On the Graph page, if there are any comments available, there will also be a Print Comments button to print the comments on a separate page.
Settings Page
Details of the Patient Account can be modified on this page to keep it current.
If necessary, Botox data can be deleted by the patient for a specified Botox injection period.
After ALL symptom data for a Botox Date is deleted, then the Botox Date will also be deleted.
Also, the entire patient account can be deleted when desired.
This can be accomplished without any support from the webmaster.
Once any Botox data or the patient account has been deleted, it can NEVER be recovered.
Doctor Account
New Doctor Page
A doctor that has patients using this website for their Botox diaries can create an account to provide easy access to view the diaries of their patients.
The doctor creates an account by entering their Name, User ID and Password (encrypted when saved).
After a doctor has created an account, a patient is able to select a doctor to give the doctor access to their diaries.
The doctor must enter their full name so a patient can choose the proper doctor to view their diaries.
No other personal information is required for the doctor account.
If the User ID is forgotten, it cannot be recovered or reset by the webmaster. If the Password is forgotten, it cannot be recovered or reset by the webmaster.

Patients Page
After a doctor signs in, they can access a patient's page to gain access to patient diaries.
Because patient accounts are only identified by their special User ID, a patient must notify their doctor which User ID is theirs.
When the doctor selects a patient to be viewed, a new web page displays the patient data.
Both List and Graph pages are available to the doctor.
The doctor cannot modifiy any patient data.
When accessing a patient's diary, there is also the ability to print the reports from both the List page and the Graph page.
Doctor Settings Page
A doctor account can be changed or deleted by the doctor without any support from the webmaster.
Changing or deleting the doctor account will have no effect on any patient account.
Once the account has been deleted, it can NEVER be recovered.
Staff Settings Page
A Doctor can manage Staff member accounts that will be able to access patient diaries currently available to the doctor.
A staff member will be able to sign in with their User ID and Password and access patient diaries in the same manner as their doctor.
Staff Account
A Staff Account must be created by a Doctor Account.
Patients Page
A staff member will be able to sign in with their User ID and Password and access patient diaries in the same manner as their doctor.
After a staff signs in, they can access only patient diaries for the doctor who created the staff account.
Because patient accounts are only identified by their special User ID, a patient must notify their doctor which User ID is theirs.
When the staff selects a patient to be viewed, a new web page displays the patient data.
Both List and Graph pages are available to the staff.
The staff cannot modifiy any patient data.
When accessing a patient's diary, there is also the ability to print the reports from both the List page and the Graph page.
Staff Settings Page
A staff account can be changed by the staff without any support from the webmaster.
Changing the staff account will have no effect on any patient account.
